Abstract:We present the case of a 61-year-old male with bilateral knee dislocation with right knee surgery in 1974, at age 18, in whom a patellar realignment was performed by reinserting the patellar tendon. He is currently diagnosed with rupture of internal collateral ligament, left patellar tendinopathy and bilateral severe knee osteoarthritis since 2013. It shows symptoms of pain and sensation of instability of the left knee, when walking for a long time (maximum that can walk are 10 - 15 minutes) the pain makes him stop the activity he’s doing (referring to activities such as walking or walking the stairs, or just going cycling). There is a deficit of joint balance at the level of the femoro-tibial joint, especially in the lower right extremity, and a decrease in the muscular balance, more in the lower left limb than in the right one. The muscular deficit it’s visible at the level of the muscular circumference of the quadriceps . After being evaluated, he receives Physical Therapy (3 sessions a week for 12 weeks) to reduce pain and correct current deficits, seeking to reduce the sensation of apprehension caused by instability when going down stairs. He has gone through masotherapy, thermotherapy, analgesic electrotherapy (TENS), stretching, exercises of analytical muscle reinforcement and proprioceptive exercises. The patient's education was also emphasized in order to avoid aggravation of current symptoms and to prevent the occurrence of symptoms in other areas (right foot pain due to poor postural habits). At the end of the treatment, we evaluated the possible changes observed and they showed an improvement in pain perception, knee instability, joint balance and muscle balance.
