Efecto de una dieta altamente hipocalórica sobre el control de la obesidad mórbida a corto y medio plazo

    Abstract:
    BACKGROUND: The ponderal evolution and biochemical parameters during the inpatient administration of a highly hypocaloric and high protein diet (VLCD) were evaluated. The efficacy at medium term of this treatment was determined and the possible determining factors of the ponderal evolution were analysed. METHODS: A VCLD was administered to 20 women with morbid obesity during 21 days. The modifications of the biochemical parameters were determined during admission. The ponderal evolution was studied during hospitalization and during 9 moths of outpatient follow up. The prognostic factors of ponderal evolution analysed were: age, attempts to lose weight, familiar history of obesity, maximum weight attained, weight and index of body mass (IMB) at study initiation and admission, loss of weight during admission and the number of outpatient controls. RESULTS: During hospitalization, a mean loss of 7,0 ± 1,9 kg, a significant decrease in glycaemia, cholesterol, triglycerides, lymphocytes and blood pressure were observed. It was found that ponderal loss at 3 and 6 months may be predicted by IBM upon initiation of the study. At 9 months weight was lower than at initiation of hospitalizatio but not than at discharge (p<0.001). Weight upon initiation of the study and the maximum reached explain that weight at 9 months in 82,5% of the cases. CONCLUSIONS: Highly hypocaloric and high protein diets lead to a significant weight loss which is maintained at medium term and an improvement in the factors of cardiovascular risk are observed. Of the factors evaluated, only the grade of obesity significantly influences ponderal evolution

Of the factors evaluated, only the grade of obesity significantly influences ponderal evolution FUNDAMENTO: Se evalúa la evolución ponderal y de los parámetros bioquímicos durante la administración hospitalaria de una dieta altamente hipocalórica e hiperproteica (VLCD), se determina a medio plazo la eficacia de dicho tratamiento y se investigan lo posibles factores determinantes de la evolución ponderal. MÉTODOS: Se administró a 20 mujeres con obesidad mórbida una VLCD durante 21 días. Se determinaron las modificaciones de los parámetros bioquímicos durante el ingreso. Se estudió la evolución ponderal durante la hospitalización y durante 9 meses de seguimiento ambulatorio. Los factores pronósticos de evolución ponderal analizados fueron: edad, intentos de pérdida de peso, antecedentes familiares de obesidad, peso máximo alcanzado, peso e índice de masa corporal (IMC) al inicio del estudio y del ingreso, pérdida de peso durante el ingreso y número de controles ambulatorios. RESULTADOS: Durante la hospitalización se observó una pérdida media de 7,0 ± 1,9 kg, una disminución significativa de la glucemia, colesterol, triglicéridos, linfocitos y presión arterial. Se observa que la pérdida ponderal a los 3 y 6 meses puede predecirse por el IMC al inicio del estudio. A los 9 meses el peso fue inferior al del inicio de la hospitalización pero no al del alta (p<0.001). El peso al inicio del estudio y el máximo alcanzado explicarían en el 82,5% el peso a los 9 meses. CONCLUSIONES: Las VCLD ocasionan una pérdida de peso significativa que se mantiene a medio plazo y una mejora de los factores de riesgo cardiovasculares. De los factores evaluados sólo el grado de obesidad influye de forma significativa sobre la evolución ponderal.
