Perfiles clínicos y variables de desenlace terapéutico en mujeres con distimia en un tratamiento grupal cognitivo-conductural

    The first objective of our thesis is to obtain clinical profiles in dysthymic women based on personality pathology and characterize these subtypes in terms of severity, type of childhood maltreatment, dysfunctional maladaptive patterns and coping strategies. The second objective is detect variables associated with less favourable outcome in psychotherapy. Our sample consists in 91 women diagnosed with dysthymia, in the second phase of the study 51 women participated in a cognitive-behavioural and schemas group therapy. In the clinical evaluation is used The Structured Clinical Interview, the Hamilton Depression Scale, the Beck Depression Inventory, the Childhood Trauma Questionnaire-Abbreviated Form, the Brief COPE questionnaire, the Young Schema questionnaire and the pathological personality assessment questionnaire of Livesley. We have found two subtypes of dysthymia. In group 1, personality dimensions are within normality, with the exception of identity problems. In group 2, unlike group1many pathological personality traits, significantly higher scores on HDRS and BDI, higher type and severity of childhood abuse, significantly higher scores in all domains of schemes except goals and grandiosity and scored significantly higher in emotional coping. In the second phase of the study we found that physical abuse is associated with less improvement in the depression scales. Vulnerability schema was negatively associated to improvement in HDRS. Dependency schema and insufficient self-control schema were associated to improvement in BDI. We find that passive cognitive coping is negatively associated with improvement in HDRS and BDI and passive emotional coping in a positive way.
