    Introduction: The aging of the population leads to an increase in morbidity and chronicity. Current care systems are characterized by the atomization of services aimed at acute pathology, resulting in excessive visits and duplicity in the management of cases. On the other hand, integrated care is able to address the needs of the current population efficiently, but more studies are needed to adapt innovative models to local care systems. Aim: To evaluate an innovative model of integrated social and health care in the health region of Terres de l'Ebre in improving the health of people with chronic diseases and their caregivers. Methodology: Mixed methodology study in 2 phases. The first phase is based on a quasi-experimental multicenter single group assignment pre-post clinical trial. An intervention has been carried out based on an innovative model of integrated care for chronic patients through an application (App Salut+Social) that promotes the coordination of health professionals and social services. The effectiveness of the intervention is measured in terms of adherence to the new model and improvement in: quality of life, adherence to treatment, quality of care, caregiver burden and access to social services. The second phase consists of a qualitative study based on a focus group of professionals and interviews with patients and carers in order to assess the acceptability, strengths and weaknesses of the model. Results: The Health+Social integrated care model achieves the coordination of health and social services. Which results in an improvement in the quality of life, adherence to treatment, access to certain social services and has eased the burden on carers. Qualitatively, it results in greater satisfaction and care efficiency for both professionals and patients. Conclusion: Integrated care improves the satisfaction of patients with chronic conditions compared to traditional care and optimizes case management.
