Utilidad del estudio intraoperatorio de márgenes en el carcinoma basocelular de cabeza y cuello

Abstract:
Basal cell carcinoma is the most frequent cancer in many countries of the world and its main location is the head and neck (70-80% of cases, according to the series). The therapeutic arsenal available to treat this tumor is broad, but surgery is the first choice. Mohs micrographic surgery is a surgical technique considered by many authors as the gold standard for the treatment of basal cell carcinoma, but its cost is high and not all hospitals have sufficient resources to perform it. The authors report a series of 124 patients diagnosed with basal cell carcinoma of the head and neck, treated by surgical excision and intraoperative study of vertical frozen sections, a method that allows the immediate evaluation of tumor surgical margins. Demographic data are collected from the patients, the method used for the histological study of tumor surgical margins is described and the results are analyzed. The mean postoperative follow-up time was 5.03 years. Based on the results obtained, it can be concluded that the method of work that we follow (tumor excision with a surgical margin of 3mm and intraoperative study with vertical frozen sections) is a very adequate therapeutic option for basal cell carcinoma of the head and neck, because offers a very low recurrence rates (<1%) and the best cosmetic results.
