    Abstract:
    Background: Obesity is a public health problem and understanding its origins, mechanisms and complications is a challenge. Ghrelin, a gastric hormone, may be involved. The objective of this study was to evaluate ghrelin-producing cells in different gastric regions in morbidly obese patients who have undergone Laparoscopic Sleeve Gastrectomy (LSG) and compare these results with clinical and pathological findings. Methods: This study included 61 morbidly obese patients undergoing LSG between 2012 and 2014. Samples from the antrum, body and fundus were processed by conventional histological and immunohistochemical studies for ghrelin-producing cells. Histopathologically, inflammation, severity, intestinal metaplasia and atrophy were analyzed, and ghrelin cells were quantified. Histopathological findings were correlated with clinical characteristics. Results: 73.8% patients were female and 26.2% were male, aged between 27 and 73 years. The mean BMI was 51.11 kg/m². 36.1% patients were diabetic, 59% presented arterial hypertension and 18% had both comorbidities. Gastritis was present in 88.5% of cases. The number of gastric ghrelin cells significantly depended on gastric location. The number of ghrelin cells in the antrum was higher in men than in women. In the hypertensive and normotensive patients, there was a significant difference in the number of ghrelin cells in the antrum. Topographically, the hypertensive group showed a significant variation in the number of positive ghrelin cells. Conclusion: Our results demonstrate that the antrum potentially produces ghrelin. They also show that the topographic ghrelin-producing cells in our patients depend on gender and comorbidities. Moreover, gastritis is often found in these patients.
