Tractament de l'insomni en el pacient amb trastorn esquizofrènic: adaptació i avaluació d'una intervenció cognitiu conductual i psicoeducativa

Abstract:
nsomnia is the most common sleep disturbance in the psychiatric field with consequences that cover the individual, work, economic and social spheres and affect the quality of life. The main objective of this thesis has been to adapt and evaluate a cognitive behavioral and psychoeducational intervention, through a randomized clinical trial, of a group intervention for the treatment of insomnia in patients with schizophrenic disorder. The secondary objectives have been to know the prevalence of insomnia and its association with the quality of life related to health and to know the experience of insomnia through a study with an ethnographic approach, in a sample of patients with insomnia and schizophrenic disorder. The results of the study showed that 63.7% of the sample presented some of the symptoms of insomnia. The prevalence of insomnia in the schizophrenic disorder population is 23.2% (CI 95% 18.5% - 28.6%) according to ICD-10, and according to DSM-IV, in 7.9% ( CI 95% 5.2% - 11.7%), The dimensions of the quality of life have shown significance of risk with respect to insomnia: mobility OR: 3.54 (CI 95% 1.88 – 6.65), personal care OR: 2.69 (CI 95% 1.36 - 5.32), activities of daily living OR: 3.56 (CI 95% 1.97 - 6.44), pain/discomfort OR: 4.29 ( 95% CI 2.37 – 7.74) and anxiety/depression OR: 3.01 (95% CI 1.61 – 5.65). Regarding the narratives, patients show differences in the intensity and experience of insomnia depending on the severity, as well as its impact on their quality of life, they describe ruminations and obsessive thoughts as a key factor that prevents falling asleep.
