Mortalidad materna a principios del siglo XXI en los Altos de Chiapas, México: Violencia estructural y búsqueda de atención

Abstract:
Maternal mortality continues to be a significant health problem in the 21st century, especially in low-income countries. Despite global advances, 99% of these deaths occur in developing countries, where the MMR is 20 times higher. This problem highlights disparities in economic and social development, as well as in the status of women, reflecting inequities among women and in access to health services. In 2001, Chiapas, Mexico, recorded 87 maternal deaths, 11 of which occurred in the Altos Region, notable for its high density of indigenous population and significant levels of monolingualism and illiteracy. Verbal autopsies revealed an additional 19 deaths that were misclassified due to a shortage of medical personnel and health resources. The World Health Organization identifies poverty, distance, lack of information, lack of adequate services and certain cultural practices as significant barriers. These elements contribute to 'structural violence,' a phenomenon rooted in poverty, social exclusion and marginalization that limit women's ability to respond effectively to obstetric emergencies.
