Resumen:
TITLE. Role of hepcidin in infants: relationship with infectious diseases, inflammatory cytokines and anemia. OBJETIVE. Study the relationship between infection, expression of proinflammatory cytokines and hepcidin levels and the development of anemia in infants. METHODS. Prospective longitudinal study with subjects aged 0-12 months, that analyzed sociodemographic, anthropometric , dietary and infectious parameters ( blood count, ferritin, hepcidin and proinflammatory cytokines ) . RESULTS. 124 subjects, 43 % (n = 54) men. 28% (n = 35) exclusively breastfeed. Median hepcidin at 6 months: 42ng / ml (range 36.65 to 54.80) and 53.27 ng / ml (range 41.56 to 73.47) at 12 months. 50% ( n = 62 ) presented infections, 32 % ( n = 39 ) bronchitis , 11% ( n = 13 ) bronchiolitis , 11% ( n = 13 ) gastroenteritis, 7 % ( n = 8 ) otitis and 5 % urinary track. There is a relationship between inflammatory cytokines (IL- 1β and TNF – β) and hepcidin levels at 6 and 12 months. At 6 months (Rho = 0.28, p0.027; Rho = 0.29 p = 0.012 respectively) and 12 months (Rho = 0.41 p < 0.001 and p0.44 Rho <0.001. There is an increase in cytokines (IL - 2 and TNF- β) and increase in hepcidin from 6 to 12 months Exclusively breastfed infants had fewer infections ( β -1.35, p 0.005 ) , IL2 undetectable ( p = 0.02 ) and lower levels of hepcidin ( 39.35 ( 29.2 to 50.1 ) vs 46 ( 37.8 to 602 ) ) p 0.034 . Lower levels of hepcidin were associated with the presence of anemia at 12 months. CONCLUSIONS. In infants IL - 1β and the TNF- β , cytokines have a regulatory role in hepcidin From 6 to 12 months there is an increase of inflammatory cytokines and hepcidin levels. Exclusively breastfed infants have fewer infections, lower hepcidin levels and undetectable levels of IL2 when compared to bottle-fed infants.
