    Resumen:
    Early hospital discharge of preterm infants favours the weight gain, the risk reduction of infections and the restoration of the affective bonding between parents-baby interrupted due to the admission of the baby in the neonatal intensive care unit. This thesis contains the process of design, development, impact and satisfaction evaluation of the e-Health follow-up program for the early discharge of the premature infant with a telematics tool called e-Prematur. Through these pages, someone can see the creation process of the program and the e-Prematur tool, form the starting point, its design, the collaboration with an IT company, its implementation, its impact and the satisfaction degree of the parents who participates in the program and nursing staff that has been in charge of the follow-up. As well as the narrative about the expectations, emotions and needs felt by the parents of preterm infants in relation with the early discharge. The Programa d’eSalut per al seguiment de l’alta precoç del nadó prematur has allowed us to respond the needs of a clinical follow-up of the premature baby and the accompaniment of their family.
