    Osísntderoopmoero msiest aisb aónlic aog. e -related chronic disease characterized by the imbalance in bone turnover that damages bone structure and declines bone resistance, increasing the risk of fragility fractures. As previous publications have suggested, diet is one of the main modifiable risk factors associated with the development of chronic diseases like osteoporosis. Moreover, as a globalization result, diet quality has shown a decrease in the last years, which increases the potential harmful effect of this factor on health. The main objective of this Doctoral Thesis was to evaluate the association between food and diet quality with bone mineral density and the risk of osteoporotic fractures in two older Mediterranean populations at high cardiovascular risk and/or with metabolic syndrome from the PREDIMED and PREDIMED-Plus studies. The results of the present Thesis show that a higher consumption of extra virgin olive oil is prospectively associated with a decreased risk of fragility fracture, whereas diets with a higher glycemic index, glycemic charge, potential renal acid load, and net endogenous acid production were associated with an increased incidence of osteoporotic fractures in an older Mediterranean population at high cardiovascular risk
