    OBJECTIVE To evaluate the effectiveness of intervention based on brief health advice against smoking, combined with detailed information on the results of spirometry, applied to male and female smokers with no history of respiratory pathology, seen in Primary Care, to improve the rates of prolonged abstinence for 6, 9 and 12 months, as well as timely abstinence. METHODOLOGY Randomised, parallel, controlled, multicentre clinical trial. Participants were smokers with a minimum cumulative consumption of 10 pack-years, between 35-70 years of age and no history of respiratory pathology. Allocation was randomised to intervention (n=308) and control (n=306). Both groups received brief advice to quit smoking. Only the intervention group underwent forced spirometry and the results were explained in detail. The primary endpoint of the study was prolonged abstinence for 12 months, biochemically confirmed by urine cotinines at the end of follow-up. MAIN RESULTS Prolonged abstinence over 12 months was 7.8% in the intervention group and 2.6% in the control (OR 3.15; 95%CI 1.45-7.59). Intervention was an independent factor for smoking cessation (OR 2.8, 95%CI 1.2-7.7), and was maintained at follow-up (HR 2.74, 95%CI 1.13-6.62). Being in the preparation or action phase according to the Prochaska and DiClemente model of change also improved rates of prolonged abstinence (HR 2.55; 95%CI 1.07-6.09). At the end of follow-up, point abstinence was 9.7% in the intervention group and 3.9% in the control group (OR 2.64; 95%CI 1.36-5.47). CONCLUSIONS In smokers aged 37-70 years, with no history of known respiratory pathology, the described intervention significantly improves the likelihood of quitting smoking.
