Introduction: Stroke is one of the main causes of death and long-term disability worldwide. Objective: To evaluate the global costs of the stroke process in the Terres de l'Ebre and according to sex. Methodology: Thesis structured from the publication of three articles: a systematic review of the global costs of stroke, a systematic review of the out-ofhospital costs of stroke, and a descriptive, multicenter and retrospective study of strokes diagnosed in Terres de l 'Ebre in order to analyze the epidemiology and costs differentiated by sex. Results: The global costs of stroke have a wide range between €610-€220,822.45 depending on the 'cost of illness'. The percentage of costs according to stage can be differentiated into primary prevention (8.8%), acute stroke (31.67%) and post-stroke (49.4%). Social costs increase by 300% in patients with residual disability. In the territory, women were older, with a greater prevalence of hypertension; greater stroke severity, longer hospital stays and were more likely to be referred to residential services at 90 days. Women have higher costs in cardiovascular risk factors, transient ischemic attacks, institutionalization and support needs. They showed lower costs for undergoing Endovascular Therapy and rehabilitation services. The mean unadjusted costs of stroke were 22,605.66 (95% CI 20,442.8-24,768.4), higher in women compared to men [p = 0.027]. Conclusions: Given the variability of costs, it is necessary to define a common evaluation system.
