Abstract:
Abrstract Objective: Elaborate care planes from actual literature about avoidable hospitalizations (AH) from ambulatory care sensitive conditions (ACSC). Design: Systematic review. Data sources: A systematic searches in the main databases from January 2009 to March 2016 to approach the AH-ACSC, the prevention and the PA cares. Research selection: Were selected a total of 47 articles and clinical practice guides. The selection standards were: Spanish documents sources, published in Spain and South America, between 2009 and 2016, which will it contain AH-ACSC and primary attention cares for: angina, COPD, cardiac insufficiency and diabetes. Data extraction: Were extracted information about AH-ACSC, cares and primary attention programs for the AH on elder person. Method: We perform a search in the databases: CUIDEN, CUIDATGE, COCHRANE Library Plus and Google Scholar. Results: We were selected a total of 30 articles which will it refer to primary attention cares about pathologies outlined above to create standard cares of each of them.
