Efficacy of action observation combined with therapeutic exercise in balance and prevention of falls in people over 65 years: pilot study

    Abstract:
    Objective: To evaluate the efficacy and feasibility of a program combining therapeutic exercise with action observation to improve static and dynamic balance and prevent falls in individuals over 65 years of age at low or moderate risk of falling. Design: A randomized, parallel-group, three-arm, single-blind pilot study lasting six weeks. Methods: Individuals over 65 years of age at low or moderate risk of falling were recruited. Participants were assigned to three groups: therapeutic exercise, therapeutic exercise with observation of a standard model, and therapeutic exercise with observation of a chosen model. The intervention was conducted over four weeks, three days per week. The primary outcomes were static balance, assessed via sway area using force platforms, and dynamic balance, assessed using the Timed Up and Go test. Secondary outcomes included muscle strength, fall incidence, and participant satisfaction. Results: Of 30 individuals assessed, 27 were randomized and 14 were included in the final analysis. No statistically significant differences were observed between groups regarding static balance, dynamic balance, muscle strength, or fall incidence. All groups reduced their Timed Up and Go completion times following the intervention, though there were no significant differences between groups. Fall incidence was low, with only one fall recorded during the intervention period. Satisfaction with the program was high. Conclusions: The combination of therapeutic exercise and action observation did not show superiority over therapeutic exercise alone. However, the program was acceptable and allowed for the identification of improvements needed for future clinical trials. Trial registration: ClinicalTrials.gov, NCT07199790.
