Abstract:
Introduction: The Peritoneal Dialysis is correlated with a better quality of life, a greater survival rate and a lower sanitary cost. We wonder why, in equal conditions of the candidates, the decision on the Renal Replacement Therapy ends up inclined mainly towards the Hemodialysis instead of the Peritoneal Dialysis. Objective: To analyse, by bibliographic review, the reasons why hemodialysis remains the main option for Renal Replacement Therapy. Methods: A narrative bibliographic review of the following databases: Cinahl, Cuiden, Dialnet and Scielo, was conducted between the months of January and March 2018. Out the 197 articles found, 7 were selected after being subjected to inclusion and exclusion criteria and 2 by reverse search. Results: The multidisciplinary team in the pre-dialysis consultation offers the patient the necessary information and education to make decisions. It is emphasized that medical paternalism is very rooted among the population, which can condition an informative bias that predisposes to choose Hemodialysis. The professionals recognize that they do not have enough training on the treatment of Peritoneal Dialysis. The choice of the different modalities of Renal Replacement Therapy is influenced by the type of provision of the health services, and their distribution and organization in the territory. Conclusions: The proposal on improving actions are: to generalise in the predialysis consultation, changes of the protocols, to improve professional training, to incorporate the figure of the expert patient and to ensure access to PD throughout the territory.
