Perspectives and experiences of the patients in the Home Care Service and their caregivers before the situation of clinical complexity.

    Abstract:
    The sustained increase in life expectancy along with the chronification of diseases has led us to a new health model and a new social environment. In this context, a new typology of citizen that entails clinical complexity arises. Consequently, this requires rethinking both the organization of health systems and health care practices. Catalonian Health System has been developed an organizational model of integral attention to chronicity. This model is integrated and focused on the citizen and empowers people to make decisions based on their principles and values referred to their own health. In order to investigate the relevant aspects of this study we propose a qualitative design that includes an explanatory and interpretative orientation and also considers it as a phenomenological approach. All of the above leads to know the expectations caused by a clinical complexity in an urban primary health center to the Home Care Service patients and their caregivers. It is also intended to give meaning to their experiences, to analyse their needs and to identify the methods to cope the disease. As a data collection instrument we will use patient semi-structured interviews in depth and discussion groups with caregivers. This study incorporates knowledge that will be essential to understand this new clinical status, will be applicable and will improve the care practice and also align clinical decisions with the values and preferences of patients and caregivers.
