    Abstract: "Introduction: Cranio-encephalic trauma (TCE) is one of the most prevalent pathologies in the world, with high mortality rates and permanent disability so it has been considered a public health problem; anemia is a prevalent condition in neuro-critical patients and red blood cell transfusions are frequent. Although transfusions with low levels of hemoglobin have been shown to be associated with equivalent or better results than higher hemoglobin thresholds, in spite of the fact that in patients with TBI, anemia may be considered to favor secondary brain injuries such as hypoxemia. Objective: To identify the thresholds of red blood cell transfusion and the neurological results associated with transfusion strategies in patients who are victims of severe cranioencephalic trauma. Methodology: We will carry out a systematic review in accordance with the declaration of preferred report elements for systematic reviews and meta-analysis (PRISMA) and the methodological recommendations of the Cochrane Manual for systematic reviews of interventions version 2.0. Results: The search in the PubMed, Web of Science and Scopus databases launched 986 studies, and 108 studies identified through additional sources. Five hundred and sixteen duplicates were removed. In the analysis, a total of 169 articles were selected from which 147 were removed for not fulfilling the purpose of the review and finally 22 studies were analyzed where he described the thresholds of transfusion and the neurological results associated with transfusion strategies in patients with severe cranioencephalic trauma. Conclusions: The current literature seems to support restrictive transfusion protocols (Hb els7 g/dl) compared to threshold liberals (Hb els10 g/dl) in patients with severe cranioencephalic trauma, however there are few studies and it is recommended to individualize each patient."
